Service-Specific, Minimum-Necessary Information
- Medical appointments and care coordination: with your explicit authorization, we may share the minimum scheduling and contact information with the clinic or provider you select to arrange, confirm, or manage an appointment.
- Patient transportation: with your explicit authorization, we may share the minimum contact information, pickup address, appointment destination, and scheduling details with the transportation provider responsible for the ride you request. We do not share health history or other private information unless it is strictly necessary for that transportation service and you expressly authorize it.
- Health coverage, food, financial, telephone, or other assistance programs: with your explicit authorization, we may share only the minimum information that the specific program or agency requires to process the assistance request you asked us to submit, such as contact or eligibility information. We will not submit a request or disclose information to a program, agency, clinic, or provider that you have not authorized.
We do not disclose information to affiliates, partners, agencies, clinics, insurers, advertisers, marketers, analytics providers, or other organizations merely because they work with us or offer a potentially relevant service.
